Please note, employees whose child is enroll in dependent life insurance only, will need to contact the Benefits Department to initiate the coverage update.  Disabled dependents can be covered beyond age 26, if additional documentation is submitted.

In the event of divorce or dissolution of domestic partnership, that individual will no longer qualify for coverage. They must be removed within 60 days of the life event through a status change request. The former spouse or domestic partner will be offered COBRA coverage, accordingly.
