Accessing the UPMC MyHealth Questionnaire

Faculty and Staff

1. Log in to the Health Plan Access account (via Pitt Passport)
2. Select Wellness for Life next to the quick links section (it may also be selected from the top of the screen under Wellness)
3. Read and agree to the terms and conditions
4. Select MyHealth Questionnaire
5. Complete the assessment

Covered Spouses/Domestic Partners

1. Visit upmchealthplan.com/members
2. Log in if you already have an account; if you do not have an account, select Register for Health Plan Access, then accept the terms, enter the requested information and follow the account setup instructions
3. Once logged in Select Wellness for Life next to the quick links section (it may also be selected from the top of the screen under Wellness)
4. Read and agree to the terms and conditions
5. Select MyHealth Questionnaire
6. Complete the assessment
