Dental Plans and Rates
Dental coverage is offered through three benefit plan options, including a very popular dental health maintenance organization (DHMO) plan and a traditional indemnity program with two "flex" plan designs.
The DHMO, Concordia Plus, includes 100 percent coverage of cleanings, exams, and other preventative services, as well as access to University Dental Health Services, a comprehensive dental practice located on the Pittsburgh campus. The traditional indemnity programs, Concordia Flex I and II, offer access to a broad network of dentists, as well as full coverage of preventative services.

Concordia Plus DHMO
- Requires selection of a network primary dental office,
- Charges no annual deductible or annual plan-year maximum,
- Covers 100 percent of the cost of cleanings, exams, and other preventative services, and
- Provides partial coverage of orthodontics for eligible dependents up to age 19.

Concordia Flex I
- Allows participants to choose dentists from a broad network, and
- Covers 100 percent of the cost of preventative services.

- Click on "find a dentist."
- For the Concordia PLUS plan, select “DHMO Concordia Plus General Dentist” from the “My Network Is” drop down menu
- For the Concordia FLEX I and II plans, select “Concordia Advantage” from the “My Network Is” drop down menu
